The following steps offer information and forms for enrolling your child into Prior Lake-Savage Area Schools. All enrollment forms are processed through our Enrollment Center located at 4540 Tower Street SE in Prior Lake. There you will also be able to make an appointment to tour your child's school. Enrollment Center office hours are 8:00 a.m. - 4:00 p.m. Monday - Friday.

Step 1 Choose the grade level in which your child will attend (fill out one form per child): 
             Preschool Screening, Preschool Registration
             Kindergarten Registration
             Elementary (Grades 1-5)
             Middle School (Grades 6-8)
             High School (Grades 9-12)
             Open Enrollment (K-12)
                         
Step 2 Review the following checklist for student
             enrollment: Click here.
              
                 Obtain a birth certificate from the State of MN
             Free or Reduced-Price Lunch application (4 pages)
  

Students

Step 3 After you have filled out the enrollment forms, you may
             mail or personally deliver them to the DSC Enrollment
             Center: 4540 Tower Street SE, Prior Lake, MN 55372.
 
             While there, you can make an appointment to visit your
             child's school. You may also email enrollment forms
             to: kgerold@priorlake-savage.k12.mn.us We
             will be in contact to verify forms have been received. 
             For more information call 952-226-0000.
 
             Please note that all course registration for Prior Lake 
             High School is done at PLHS after enrollment forms 
             are submitted.
 
             All school supply lists and student handbooks are
             listed on individual school websites which can be
             accessed from the district home page.
